Hi there,
if you are graduate assistant for this fall term, this email is for you. if
not, please discard it.
all graduate students with GTA/GRA have to sigh up the Graduate Assistant
Insurance before Oct 1, if you choose use this insurance.
the insurance will *NOT* automatically continue to cover for this fall term.
please go to health center for enrollment form.
besides the enrollment form, a waiver form for International student
insurance has to be turned in also. if you can sigh up this waiver form,
$349 will be charged on your account.
so please remember to sign up these two forms today.
